Procedures for New JIFSAN Interns

Congratulations on your JIFSAN internship! There are several forms that must be filled out before your internship can begin. These forms can all be downloaded from our web site at http://www.life.umd.edu/internship/forms.html. Hard copies can also be obtained from Kaci Thompson (1313 Symons Hall, University of Maryland) or Christine Hileman (FDA, Center for Food Safety and Applied Nutrition).

Step 1.
Complete required forms and have fingerprints taken.

JIFSAN Volunteer Service Agreement Temporary Staff / Student Laboratory Project Safety Form	The student should work with the JIFSAN mentor to fill out these forms. The laboratory project safety form is only required for projects involving laboratory research. The JIFSAN mentor will let the student know if it required in their case.
FDA Security Card Access Request	Required for obtaining an ID badge. The student should work with the JIFSAN mentor to fill out this form.
Standard Form 85 Form 306	All potential JIFSAN interns must undergo a Security Background Check prior to beginning work. The security clearance process is quite lengthy, so the sooner the paperwork is completed, the sooner the internship can begin. Contact Christine Hileman at the FDA (301-436-1674) to drop-off security forms and arrange fingerprinting appointments.
CFSAN Supplemental Student Information Form HHS Applicant Background Survey Liability Release Agreement	The student should work with their JIFSAN mentor to fill out these forms.

Step 2.
Return completed forms, with all original signatures, to C. Hileman for processing.

Step 3.
ID Badge Photo
ID badge photo appointments are arranged through C. Hileman when all forms have been turned in for processing.

Step 4.
Pick up ID Badge Photo
Allow approximately one week for ID Badge processing. You will be notified by C. Hileman when your ID Badge is ready to be picked up.

Step 5.
Volunteer Commitment Verification Form
Use this form to keep track of the hours you have worked. Once you have completed 100 volunteer hours, you are eligible for a paid internship. Paid internship application forms are available at http://www.life.umd.edu/internship/forms.html and should be submitted to Kaci Thompson (1313 Symons Hall).
